Required for all minor participants (athletes under 18 years of age) competing in the National Women's Throwball Championship.
Authorizes your child to travel for the tournament, including any international travel if applicable, confirming all required documents are in order.
Confirms your child has appropriate travel/medical insurance. Permits tournament officials to arrange emergency medical care if needed.
Acknowledges the inherent risks of sports participation and releases the Federation from claims arising from injuries or damages during the event.
Confirms your child will follow all tournament rules, safety guidelines, and codes of conduct set by the organizers throughout the event.
Important: This form must be signed by the lawful parent or legal guardian of the minor athlete. The minor's full name, date of birth, passport number, and nationality are required fields. A minor will not be permitted to participate without a completed and signed consent form on file.
Required for all registered participants in the TFUSA National Women's Championship 2026. Minors require an additional parent/guardian signature.
Confirms you understand and voluntarily accept the inherent risks of participation including physical exertion, contact, and facility conditions.
Releases TFUSA, its officers, volunteers, sponsors, and venue operators from all claims related to injury, illness, property damage, or death.
Grants TFUSA the right to capture and use your name, image, and likeness in photographs and videos for promotional and archival purposes.
You agree to hold harmless and indemnify the Releasees from any claims arising from your participation or any breach of the waiver terms.
Important: Please read the entire waiver carefully before signing. All fields — participant name, signature, date, and emergency contact — are mandatory. If the participant is under 18 years of age, a parent/guardian signature is also required in addition to the Parent/Guardian Consent Form above.
